- Performance fating in the last rating period (if applicable);
- Photocopy of certificate of eligibility/rating/license;
- Photocopy of Transcript of Records;
- Photocopy of Training Certificates; and
- Photocopy of Certificate of Employment/Service Record from previous employers.
